Title:Hepatic morphology in reactional states of leprosy.

Abstract:Liver function tests and liver biopsies were studied in 23 leprosy patients in reaction and 10 without reaction. The liver biopsies in leprosy patients with reaction showed exudative lesions, epithelioid and tuberculoid granulomas, and foam-cell granulomas. Portal vasculitis was encountered in a few cases. Neutrophilic infiltration into the foam-cell granulomas was seen in a few cases of lepromatous (LL) leprosy with reaction. In six cases of borderline (BL, BB and BT) leprosy with reaction, a spectrum of lesions bearing footprints of exudative lesions were seen evolving into epithelioid-cell granulomas. Foam-cell granulomas and tuberculoid and epithelioid granulomas along with exudative lesions were encountered in two cases on individual biopsy strips. An altered albumin-to-globulin ratio was the chief functional derangement observed in these cases. The spectrum of changes observed in borderline leprosy with reaction could be discrete steps in the evolution of upgrading reaction^ien.

Title:Studies of human leprosy lesions in situ using suction induced blisters 1. Cellular components of new, uncomplicated lesions.

Abstract:The cellular contents of blisters induced by suction over new, uncomplicated leprosy lesions, and in the skin of cured, control patients, have been examined with enzyme- and immuno-histochemical staining over a period of 4 days. The total cellularity of the blisters varied over a wide range, not correlated with the type of leprosy. Mononuclear cells predominated at all times studied, with nearly equal percentages of monocytes and T lymphocytes. The T-helper: suppressor ratio was significantly greater in BT than in BL and LL lesions at 48 hr. Suction blisters offer a painless, quantitative, reproducible, multiple-sampling method for obtaining cells from the cutaneous infiltrates of leprosy for phenotyping or functional analysis^ien.

Abstract:Visando contribuir para o melhor entendimento da participação das citocinas na hanseníase dimorfa, o presente estudo investigou a produção desses mediadores in vitro e in situ em pacientes dimorfos-tuberculóides (HDT) e dimorfos-virchovianos(HDV). Foram avaliados 7 pacientes HDT e 12 HDV, virgens de tratamento, além de 19 indivíduos sádios (grupo controle). Culturas de células mononucleares do sangue periférico (PBMC) foram estimuladas ou não com estímulos inespecíficos e específicos do M. Leprae (antígeno inteiro e sonicado) e após 48 horas o sobrenadante foi recolhido para dosagens das citocinas TNF-a, IFN-y, IL-10 e TGF-B1. Biópsias das lesões cutâneas foram submetidas aos procedimentos histológicos por meio da coloração com hematoxilina-Eosina e Faraco-Fite; os cortes foram submetidos, ainda, à detecção in situ de iNOS, IL-10 e TGF-B1 por imunoistoquímica. A quantificação de citocinas em sobrenadante de PBMC revelou que pacientes HDT produziram níveis menores de TGF-B1 e pacientes HDV, níveis menores de IL-10...(AU)^ipt.

Abstract:(Role of temperature, microtraumatisms by elongation or subluxation and canalar stricture.) Cold has a slight aggravating effect on leprosy which is observed in some countries as Iran, South Russia, North India and the Andean cordillera. The subluxation of the ulnar nerve has facilitating effect in the development of the neuritic damage in a limited number of patients. The elongation is important for the production of the ulnar neuritis. The canalar stricture is a major factor causing nerve damage in leprosy. It explains the segmentary localization of the principal damages of the leprous nerves in the proximal areas situated above the tunnels. Intraneural hypertension appears first and is responsible for the swelling of the nerve which gets entraped inside the tunnel. A circulus viciosus is, then, created which maintains and aggravates the intraneural hypertension.

Abstract:Um homem de 31 anos de idade procurou o serviço de saúde especializado devido a alterações de sensibilidade em braço esquerdo. O exame clínico deu suporte a esta queixa. Os esfregaços obtidos a partir da linfa colhida nesta área não desmontraram bacilos álcool-ácido-resistentes (BAAR). A reação de Mitsuda foi negativa e uma biópsia do local não mostrou bacilos ou reação inflamatória. Mesmo assim optou-se pelo diagnóstico de hanseníase indeterminada, e foi instituída a poliquimioterapia para paucibacilar. Nove anos após, o paciente retorna apresentando várias lesões de padrão dimorfo, com quadro histopatológico característico e baciloscopia do esfregaço e da biópsia positiva. Os autores consideram que este indivíduo já era um multibacilar por ocasião do primeiro diagnóstico em 1997 e o tratamento insuficiente não conseguiu destruir todos os bacilos. Nesta situação, os bacilos remanescentes poderiam ter proliferado lenta e progressivamente, reproduzindo o longo período de incubação próprio dos indivíduos Mitsuda negativos. Discutem-se as limitações do diagnóstico exclusivamente clínico e baseado no número de lesões específicas.(AU).

Abstract:Interrelationship among six patient characteristics recorded upon entry the trial were analyzed for 67 patients with lepromatous and near-lepromatous leproay admitted into two chemotherapy trials. Sex was found to be significantly associated with age and with the histopathologic classification; disproportionately large numbers of older patients and of patients classified as borderline-lepromatous (BL) were males. Classifications of the disease process by clinical and histopathologic criteria were closely associated, but many patients classified BL on histopathological grounds were classified fully lepromatous by the clinical criteria. Measurments of the number of Mycobacterium leprae in the patients made by there methods were also significantly correlated. No significant correlations were found between either classification of the disease process on the hand, and any of the measurments of the numbers of organisms on the other (AU).

Abstract:Some mycobacterial infections, such as tuberculosis, are characterized by apoptosis in infected or by-stander mononuclear immune cells. For localized (paucibacillary, PB) and diseminated (multibacillary, MB) leprosy, characterized by polarized Thl-like vs, Th2-like immune responses, respectivelly little is known about lesional apoptosis. We analyzed sections of paraffin-embedded, untreated leprosy lesions from 21 patients by an indirect immunofluorescent terminal deoxynucleotide-transferase-mediated dUTP-digoxigenin nick end labeling (TUNEL) assay. Some TUNEL (+) PB sections were then reacted with phycoerythein-conjugated (red)antibodies against T cells, monocytes, or antigen-presenting (Langerhans) cells. TUNEL (+) bodies were detected in 9 of 16 PB lesions (56%) and in 1 of 5 MB lesions (20%). Some TUNELL (+) bodies in PB disease were CD3+ (T cell), as well as CD4+ (T-helper) or cd8+ (T-cytotoxic) Apoptosis characterizes PB and MB leprosy lesions and may be more freqeunt in PB disease. In PB disease, some TUNEL (+) bodies may derive from T cells (AU).

Abstract:Um paciente de 54 anos, com o diagnóstico de hanseníase dimorfa internado no Hospital para tratamento fisioterápico, apresenta quadro infeccioso sistêmico de evolução subaguda, associado a manifestações inflamatórias agudas nas extremidades superiores com comprometimento ósseo. Este comprometimento caracterizou-se por lesões tipo periostite em falanges e metacarpos a áreas sugestivas de erosões ósseas. Falece em colapso tóxico-infecciosos e insuficiência cardíaca congestiva. A autopsia evidencia Endocardite bacteriana trombo-ulcerativa e comprometimento de pele, nervos periféricos e tecido ósseo por granulomas tuberculoides onde a baciloscopia foi positiva (+). Ainda no tecido ósseo, principalmente em localização subperiosteal há fibrose cicatricial, desorganização e neoformação óssea. È discutida a relação entre o processo de Endocardite bacteriana e a reativação da Hanseníase dimorfa .

Abstract:E apresentado um caso de hanseniase pre-dimorfa tratada com PQT/PB/OMS com alguma melhora na ocasiao da alta. Um ano apos, a area de anestesia que apresentava no dorso do pe direito se estendeu pela panturrilha ate o cavo popliteo. Seis anos depois paciente retorna ao Centro de Saude com lesoes em placa quase planas roseo-hipocromicas anestesicas no tronco e pe esquerdo. Uma biopsia de uma lesao do abdome mostrou um quadro histopatologico compativel com reacao tipo 1 e baciloscopia igual a +++. Oa autores discutem a possibilidade dos bacilos estatem em um estado de persistencia e nao terem sido destruidos pelas drogas utilizadas. Segundo eles, apos a alta houve progressao dos bacilos pelos nervos determinando alteracoes no seu microambiente e consequentemente causando aumento da area anestesica sem serem reconhecidos ainda pelo sistema imune. Quando 6 anos depois eles se multiplicaram e foram finalmente reconhecidos pelas defesas do organismo, foram em parte destruidos, e deram lugar ao aparecimento de antigenos que desencadearam uma reacao de hipersensibilidade (reacao tipo 1). Os autores chamam a atencao que esses fatos tem que ser levados em consideracao no tratamento de pacientes nessas condicoes porque a resposta ao tratamento dos casos paucibacilares e diferente dos casos virchovianos (AU).

Title:Paciente com reacao tipo I e tipo II e dez anos de seguimento / Type I and type II reactions in a patients with ten years of follow-up

Abstract:Relata-se o caso de um individuo do sexo masculino, de 52 anos de idade, que desde 20 anos atras vinha apresentando sinais e sintomas de hanseniase multibacilar, mas so procurou tratamento apos 5 anos, quando apresentava manifestacoes de Eritema Nodoso Hansenico (ENH - Reacao tipo 1) inclusive com comprometimento articular. Instalado o tratamento (PQT/MB) o paciente passou a apresentar episodios de ENH, que se continuaram apos a alta medicamentosa alternando-se ou em concomitancia com episodios de reacao tipo 1 (reacao reversa) o que definiu como dimorfo. Assim permaneceu quase 10 anos, tendo apresentado, por algum tempo, esplenomegalia e sinais de hiperesplenismo. So melhorou, quando a deteccao de bacilos viaveis levou a reinstalacao da PQT. A discussao do caso ressalta alguns aspectos interessantes desta evolucao: 1) a demora no diagnostico leva pacientes dimorfos a adquirirem caracteristicas virchovianas com rica baciloscopia (virchovianos sub-polares); 2) estes pacientes tem maior possibilidade de albergarem bacilos persistentes que eventualmente se multiplicam e estimulam reacoes tipo I; 3) a alternancia de reacoes tipo 1 e tipo 2 pode indicar a participacao da imunidade celular no desencadeamento do ENH, onde a reacao granulomatosa romperia os infiltrados especificos regressivos, expondo antigenos intracelulares. Frente ao estado de hipersensibilidade humoral, haveria deposicao de complexos imunes e desencadeamento de reacao inflamatoria aguda; 4) a alta da PQT nao significa cura da hanseniase. (AU).

Abstract:After having reviewed the contradictory opinions about the importance of hitopathological examination, the author states that it is undoubtedly on of the best methods in the treatment of leprosy if not the best, provid that the doctor makes a careful selection of the lesion to be examined. In his article he proposes simply to set forth his opinions as to how histopathology shoulb be utilized in the treatment of leprosy. He first of all takes up the question of <> leprosy, pointing out that with the histopatologic system one must confine oneself to confirming the clinical giagnosis of leprosy or to checking a possible evolution towards some other type of the disease. As regards the histopathology of tuberculoid leprosy, after commenting on the various types of lesion found in this kind of leprosy, the author concludes that to interpret the lesion of tuberculóid leprosy in the light of pathology we have to think of an allergic reaction of the reticuloendothelial system...(AU).

Title:Mast cell subsets and neuropeptides in leprosy reactions ..-

Abstract:The immunohistochemical identification of neuropeptides (calcitonin gene-related peptide, vasoactive intestinal polypeptide, substance P, alpha-melanocyte stimulating hormone and gamma-melanocyte stimulating hormone) quantification of mast cells and their subsets (tryptase/chymase-immunoreactive mast cells = TCMC and tryptase-immunoreactive mast cells = TMC) were determined in biopsies of six patients with leprosy reactions (three patients with type I reaction and three with type II). Biopsies were compared with those taken from the same body site in the remission stage of the same patient. We found a relative increase of TMC in the inflammatory infiltrate of the reactional biopsies compared to the post-reactional biopsy. Also, the total number of mast cells and the TMC/TCMC ratio in the inflammatory infiltrate was significantly higher than in the intervening dermis of the biopsies of both periods. No significant difference was found regarding neuroptide expression in the reactional and post-reactional biopsies. The relative increase of TMC in the reactional infiltrates could implicate this mast cell subset in the reported increase of the immune response in leprosy reactions. (AU).

Abstract:The sites of expression of vascular endothelial growth factor (VEGF) and of KDR, its endothelial cell receptor, were investigated in leprosy reaction Type 1, or reversal reaction (RR), by immunohistochemistry and in situ hybridization. In comparison with nonreactional leprosy, overexpression of both VEGF and KDR was seen in granuloma cells, especially epithelioid and foreign body-type giant cells, the epithelium and the vascular endothelium of RR specimens. In granuloma cells, hybridization for VEGF was stronger than immunostaining, a finding that may reflect the rapid turnover of VEGF in an immunologically dynamic situation such as RR. In the epidermis, double immunohistochemistry revealed VEGF overexpression in CDla-positive dendritic cells. The VEGF may not only be relevant for hyperpermeability and mononuclear cell differentiation (the key morphologic features in the acute, clinically evident phase of RR), but it could also be implicated in RR onset, when dendritic cells are activated in response to antigen stimulation.
